Numb hands at night: causes and solutions | Chiro Ste-Rose

Numb hands at night

Numb hands at night are often caused by poor sleeping posture, nerve compression in the cervical spine or carpal tunnel syndrome. In the majority of cases, identifying the cause can restore tingling-free sleep through simple adjustments or appropriate neuro-musculo-skeletal care.

Two o'clock in the morning. You open your eyes in the dark. Your right hand is unresponsive. Tingling invades your fingers like a silent electric shock. You shake your hand, rub it, wait for the sensation to return. It lasts thirty seconds. A minute. Then, slowly, life returns to your fingers.

If this sounds familiar, you're not alone. Nocturnal numbness of the hands is one of the most frequent reasons for consultation at our Laval clinic. And the scenario is almost always the same: people endure the situation for months - sometimes years - telling themselves that «it's nothing».

The problem is, your body is sending you a message. And that message deserves to be heard.

Why does it happen mostly at night?

It's a legitimate question. Why do your hands work perfectly all day, but betray you as soon as you put your head on the pillow?

The answer can be summed up in three words: release, position, pressure.

When you sleep, your body relaxes completely. Your muscles lose their voluntary tone, and your joints adopt positions you would never assume awake. Your wrist bends under the weight of your head. Your shoulder compresses the nerves in your arm. Your neck bends at an angle, narrowing the spaces through which your cervical nerves pass.

Whether you're standing or sitting, gravity and constant movement keep your blood and nerves circulating. At night, everything grinds to a halt. Blood circulates more slowly. Nerves, already slightly irritated during the day, become trapped in positions of prolonged compression.

This is why tingling typically appears in the second half of the night - after several hours in the same position.

Numb hands at night

Cause nᵒ 1: the sleeping posture (the famous «dead arm»).

Let's start with the simplest - and most reassuring - cause.

Do you sleep on your stomach with your arm under your pillow? Do you fall asleep on your side with your wrist tucked under your cheek? Are you the type to wedge your hand between your knees?

Each of these positions creates direct mechanical compression on the nerves in your arm. The ulnar nerve (the one that runs along the elbow) and the median nerve (the one that runs across the wrist) are particularly vulnerable when your arm remains motionless for hours on end.

The result: you wake up with a «dead» hand, intense tingling, sometimes even pain all the way up to the shoulder. The good news is that this type of numbness usually disappears within a few minutes once you change position.

Signals that point to posture

Your symptoms change hands depending on your sleeping position. Numbness disappears completely within five minutes of waking. There's no tingling during the day. And the problem improves when you sleep on your back.

💡 Key takeaway

If the tingling comes back every night - regardless of your position - the cause is probably elsewhere. Read on.

Cause nᵒ 2: what if the problem came from your neck?

This is the cause most people never suspect. And yet, it's the one we most often identify in the clinic.

The nerves that control the sensitivity and movement of your hands don't originate in your wrists. They originate in your neck, at cervical vertebrae C5, C6, C7 and C8. These nerve roots emerge from your spine, cross your shoulder, run down your arm and branch out into your fingertips.

That's a journey of more than 80 centimetres. And any point of compression along that path can cause numbness in your hands.

How a cervical vertebra can make your hand numb

When a cervical vertebra loses its normal mobility - due to prolonged poor posture, old trauma or simply natural degeneration - the space through which the nerve passes narrows. During the day, constant movement maintains mobility. At night, when everything stands still, compression increases.

Studies published in the medical literature show that nerve compression at the cervical level can produce symptoms very similar to those of carpal tunnel, including nocturnal tingling and loss of dexterity. This is what medical professionals call «double crush syndrome» (source: National Library of Medicine - Double Crush Syndrome). »

Cervical disc herniations are a frequent cause of this compression. When the disc between two vertebrae shifts and puts pressure on a nerve root, numbness can travel from the neck down to the fingers. If you're also experiencing neck stiffness or shoulder pain, it's worth exploring this avenue with a professional.

🎯 Pro tip

Cervical numbness often affects specific fingers: the thumb and index finger (C6), the middle finger (C7), or the ring and little fingers (C8). Identifying which fingers are affected helps locate the vertebral level involved.

Cervical signals

The numbness affects specific fingers (often the thumb and index finger, or the ring and little fingers - depending on the nerve root involved). You may also feel stiffness or pain in the neck. Symptoms worsen after long hours in front of a screen. And turning your head in certain directions increases the tingling.

Cause nᵒ 3: carpal tunnel syndrome

It's the cause everyone thinks of first - and rightly so, since it's one of the most widespread neuro-musculo-skeletal disorders in North America.

The carpal tunnel is a narrow passage located in the heel of your hand, at wrist level. Inside this tunnel pass nine tendons and a crucial nerve: the median nerve. When the space narrows - due to tendon inflammation, water retention or repetitive movements - the median nerve becomes compressed.

Why are symptoms worse at night?

When you sleep, your wrists naturally relax into flexion. This position folds the carpal tunnel in on itself, a bit like bending a garden hose - the passageway narrows and pressure on the nerve increases. This is why carpal tunnel tingling typically appears in the second half of the night, waking up around 3am.

Signs pointing to carpal tunnel

Tingling mainly affects the first three fingers (thumb, index and middle fingers). Shaking your hand provides temporary relief. You feel clumsy when grasping small objects. And symptoms worsen with repetitive manual activities during the day (keyboarding, driving, housework).

Carpal tunnel or cervical spine? The distinction is important

Here's a little-known fact: carpal tunnel and neck compression can produce almost identical symptoms. According to some studies, between 33 and 50 % of people with carpal tunnel also experience nerve compression in the neck.

That's why a complete examination of the cervical spine AND wrist is essential before making a diagnosis. Focusing only on the wrist when the problem is in the neck means treating the symptom without addressing the cause.

3 simple tips for a better night's sleep

While you wait for your consultation, here are three adjustments you can make this evening to reduce nocturnal tingling.

1. Keep your wrists in a neutral position

Avoid sleeping with your wrists folded under your pillow or head. If you tend to fold your hands naturally during sleep, a light wrist orthosis worn at night can maintain a neutral position and prevent compression of the median nerve. It's simple, but many of our patients in Laval report improvement from the very first night.

2. Re-evaluate your pillow

A pillow that is too high or too flat alters the angle of your cervical vertebrae during sleep. Your neck should maintain its natural curve - neither too flexed nor too extended. Do you sleep on your side? The pillow should fill exactly the space between your shoulder and your head. Do you sleep on your back? A lower pillow that supports the cervical lordosis without forcing it is preferable. At Clinique Chiro Ste-Rose, we offer a shoulder measurement service to help you choose the right pillow for you.

3. Do these stretches before going to bed

Two simple exercises can make a noticeable difference. First, median nerve glides: hold your arm out in front of you, palm up, then slowly bend and unfold your fingers ten times. Next, cervical tilt: gently bend your ear towards your shoulder (without lifting the shoulder), holding for ten seconds on each side. These movements help to clear the spaces where the nerves circulate and prepare your body for rest.

💡 Key takeaway

These three tips will alleviate the symptoms, but do not replace a professional examination. If your nocturnal tingling persists for more than two weeks despite these adjustments, the cause is probably deeper.

When should you consult a professional?

Occasional tingling after a night in an awkward position is not usually cause for concern. On the other hand, certain signals deserve prompt attention.

Consult your doctor if your numbness recurs more than three nights a week. If tingling persists for more than fifteen minutes after waking. If you notice a loss of strength in your hand - objects slipping, difficulty turning a wrench. If numbness also starts to appear during the day. Or if neck or shoulder pain accompanies the tingling.

Numbness that is treated early usually resolves itself within a few weeks with proper care. Numbness that is ignored for months can lead to nerve damage that is more difficult to correct later on.

The chiropractic examination assesses the mobility of your cervical vertebrae, tests your reflexes and checks the sensitivity of your hands. This comprehensive assessment helps determine whether the problem originates in the neck, wrist or a combination of the two - and, most importantly, identifies the approach best suited to your situation.

Your first step

Tonight, try sleeping on your back with your wrists in a neutral position and a pillow that respects the curve of your neck. Take note of any changes in tingling. This simple observation will give you a valuable indication of the origin of your problem.

Frequently asked questions

Are numb hands dangerous at night?

In most cases, nocturnal tingling is not dangerous in itself - it's your body's way of signalling temporary nerve compression. The real risk is letting it linger. Prolonged, untreated nerve compression can lead to permanent loss of feeling or muscle weakness in the hand. If numbness recurs, have the situation evaluated.

How can I tell if it's carpal tunnel or a neck problem?

Carpal tunnel typically affects the first three fingers (thumb, index and middle fingers) and is accompanied by clumsiness of the hand. A cervical problem can affect any finger - depending on the nerve root involved - and is often accompanied by neck or shoulder pain. A thorough neuro-musculo-skeletal examination can help to distinguish between the two.

Is it normal to have numb hands at night during pregnancy?

Pregnancy favors the onset of carpal tunnel syndrome, due to water retention that compresses the median nerve. These symptoms generally resolve after delivery. If the tingling becomes very uncomfortable, appropriate chiropractic care and the use of a night orthosis can provide relief.

Which pillow should you choose if your hands go numb at night?

The ideal pillow keeps your neck in its natural curve without creating excessive flexion. For side sleepers, it should bridge the gap between shoulder and head. For back sleepers, a lower pillow with good cervical support is preferable. Avoid thick pillows that push your head forward.

Can a chiropractor help with numb hands at night?

The chiropractic approach aims to restore mobility to cervical vertebrae that may be compressing the nerves responsible for tingling. Restoring movement to the vertebral segments involved, combined with targeted exercises, can in many cases significantly reduce nocturnal numbness. At Clinique Chiro Ste-Rose, SpineMED neurovertebral decompression technology is also available for cases involving herniated cervical discs.
